Tanggung Jawab
- Conduct psychosocial assessments and develop intervention plans for diverse client populations
- Provide counseling, crisis intervention, and advocacy services to individuals and families
- Collaborate with community organizations, healthcare providers, and government agencies
- Maintain accurate client documentation and case records using electronic systems
- Facilitate access to essential resources including housing, employment, and healthcare
- Participate in multidisciplinary case conferences and team meetings
- Conduct home visits and community outreach to ensure client well-being
Kualifikasi
- Bachelor's degree in Social Work or related field (Master's preferred)
- Minimum 2 years of experience in case management or social services
- Valid social work registration/certification in Singapore
- Strong knowledge of community resources and social service systems
- Excellent communication, documentation, and interpersonal skills
- Cultural competence and sensitivity to diverse populations
- Ability to work independently while collaborating effectively in teams
