Tanggung Jawab
- Conduct comprehensive psychosocial assessments of elderly residents to develop personalized care plans
- Provide individual and group counseling to address emotional and adjustment challenges
- Coordinate discharge planning and facilitate seamless transitions to community resources
- Collaborate with interdisciplinary healthcare teams to ensure holistic resident care
- Advocate for residents' rights and facilitate access to social services and benefits

Kualifikasi
- Bachelor's degree in Social Work or related field (Master's preferred)
- Valid registration with Singapore Association of Social Workers (SASW)
- Minimum 2 years geriatric social work experience in healthcare setting
- Strong assessment and crisis intervention skills
- Knowledge of Singapore's eldercare policies and community resources
