Tanggung Jawab
- Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the clinical team.
- Provide direct home care services, including wound dressing, medication administration, and symptom management.
- Offer consultative advice and emotional support to patients and their families regarding care needs and end-of-life planning.
- Monitor patient progress and vital signs, ensuring all clinical outcomes meet hospice standards.
- Document all care activities, observations, and interventions accurately in electronic health records.
- Liaise effectively with physicians, social workers, and other healthcare professionals to ensure coordinated care.
Kualifikasi
- Registered Nurse (RN) with current registration with the Singapore Nursing Board (SNB).
- Diploma or Bachelor’s degree in Nursing (RN/RN (Basic) qualification).
- Minimum of 2 years of working experience in nursing, preferably in home care or palliative care.
- Strong interpersonal and communication skills, with the ability to engage with patients and families sensitively.
- Valid Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification.
