Tanggung Jawab
- Provide comprehensive nursing care and clinical procedures in a home setting, ensuring adherence to safety protocols.
- Assess and manage patients with chronic diseases, including diabetes, hypertension, and respiratory conditions.
- Assist patients with Activities of Daily Living (ADLs) to promote independence and improve quality of life.
- Offer palliative care advice, symptom management, and emotional support to patients and their families.
- Educate patients and caregivers on health maintenance, medication management, and wound care techniques.
- Monitor patient progress and maintain accurate, timely documentation of clinical records and care plans.
- Collaborate with physicians and other healthcare professionals to ensure coordinated care.
Kualifikasi
- Registered Nurse (RN) qualification with valid registration with the Singapore Nursing Board (SNB).
- Minimum of 1-2 years of relevant nursing experience, preferably in Home Care or Community Nursing settings.
- Strong knowledge of chronic disease management, palliative care, and geriatric nursing principles.
- Excellent communication and interpersonal skills to build rapport with patients from diverse backgrounds.
- Ability to work independently and manage time effectively in a non-hospital environment.
- Valid Basic Life Support (BLS) and Advanced Life Support (ALS) certifications are highly desirable.
