- Conduct comprehensive reviews of patient medical cases, ensuring accuracy and completeness of information.
- Provide expert care coordination and guidance to patients regarding their treatment plans, medications, and health management.
- Educate patients and their families on health conditions, preventative care, and available healthcare resources.
- Ensure strict compliance with clinical protocols, regulatory requirements, and company policies.
- Collaborate effectively with healthcare providers and interdisciplinary teams to optimize patient care outcomes.
- Document all patient interactions, assessments, and care plans meticulously within designated systems.
- Utilize strong clinical judgment and critical thinking to identify potential issues and recommend appropriate interventions.
- Participate in ongoing training and professional development to maintain up-to-date knowledge of healthcare trends and practices.
Kualifikasi
- Must possess an active and unrestricted US Registered Nurse (USRN) license.
- Bachelor of Science in Nursing (BSN) or equivalent from an accredited institution.
- Minimum of 2-3 years of clinical experience, preferably in a hospital, BPO healthcare account, or a related care coordination role.
- Exceptional verbal and written communication skills in English, with the ability to articulate complex medical information clearly.
- Demonstrated strong critical thinking, problem-solving, and decision-making abilities.
- Proficiency in using electronic health records (EHR) systems and other relevant healthcare technologies.
- Ability to work effectively in a fast-paced, high-volume environment while maintaining attention to detail.
- Commitment to patient privacy (HIPAA) and ethical healthcare practices.
